| Delivery to Event, Set Up, Up to 3 continuous hours, Tear Down & Cleaning of Fountain, Chocolate of your choice, 10 lbs. Chocolate with Small Fountain, 20 lbs Chocolate with Medium Fountain, 25 lbs Chocolate with Large Fountain, Wooden Skewers and one (1) Attendant for small fountain, possibly two (2) if using medium or large fountain, drip guard, and topper of your choice. Additional Chocolate $20.00 per 5 lbs. (5 lbs of Chocolate serves approximately 50 guests.) $25.00 per hour after 3 hours for each attendant. (not included setting up or tearing down) OnaViRe’s Chocolate Fountains can provide dipping items at $1.50 per guest. (Includes 2 fruits and 2 non-fruit, plates, napkins, and serving trays for dipping items per event.) An additional charge of $17.50 will be added for each attendant per hour for travel outside of Odessa/Midland area. **ANY UNPAID BALANCE MUST BE PAID 2 WEEKS PRIOR TO THE DATE OF THE EVENT!** Total Fee agreed upon by Client and OnaViRe’s is $_____________Plus Sales Tax OnaViRe’s is holding a tentative reservation for Clients event until 5 days after the below “Contract Date”. To confirm this agreement, return one signed and completed copy of this contract along with a Deposit of $100.00. THIS DEPOSIT IS ONE-HALF (1/2) REFUNDABLE IF CLIENT CANCELS TWO MONTHS BEFORE DATED EVENT. NO DEPOSIT WILL BE REFUNDED AFTER THAT TIME PERIOD. The balance is due no later than 14 days prior to the date of the event and can be paid in Cash, Check, Money Order, Visa, MasterCard, Discover, or American Express Credit Cards. Checks should be made payable to OnaViRe’s Chocolate Fountains. There will be a $30.00 fee charged for all returned checks. If cancellation is initiated by the Client then one-half (1/2) of the deposit will be refunded if canceled two months before dated event. There will be no refunds after this time period. Only one-half (1/2) of the deposit will be refunded due to the lost of possible rental for another booking on that date. The liability of OnaViRe’s Chocolate Fountains is to perform said services is subject to proven detention by sickness, accidents, or any other actions not under our control. Upon called to an event within a 48 hour period, a sur-charge of $50.00 will be applied. OnaViRe’s Chocolate Fountain will require access to the desired set up area at least 45 minutes prior to the scheduled start time and 45 minutes after the conclusion of our service, for set up and take down. Client is responsible for providing a sturdy, level table capable of supporting the Chocolate Fountain, Tablecloths for Tables, Trays & Dipping Food Items, Plates, and Napkins. OnaViRe’s Chocolate Fountains requires at least one 120 volt circuit within 10 feet of the desired set up area and a trash container. Client accepts full responsibility and is liable for any damages, injuries or delays that occur as a result of failure to comply with these provisions. This agreement and the contents hereof represents the only warranties, express or implied, between the parties, including any implied warranty or merchantability of fitness for the particular purpose and for any other obligation or liability on the part of OnaViRe’s Chocolate Fountains. OnaViRe’s Chocolate Fountain shall not be liable for any injury, loss or damage directly or consequently arising out of the use or inability to use the Chocolate Fountain, whether used singularly or in conjunction with any other equipment. Client will indemnify OnaViRe’s Chocolate Fountain against, and hold OnaViRe’s Chocolate Fountain harmless from all claims, actions, proceedings, costs, damages, and liabilities, including attorney fees, arising out of, connected with, or resulting from the use of the Chocolate Fountain. |
